This article explains how to create custom forms that users can fill out when creating or completing work orders in the system. These forms allow you to collect specific information through custom fields, streamlining your workflow and ensuring you gather all necessary data.

Creating Forms
- Navigate to Admin > Forms and click "Create Form" on the top right
- Name the form and click "Add Custom Field" to add questions to the form
- If this is your first time creating a form, click "New Field." This will allow you to create a question. As you create forms, you will be able to then click "Existing Field" to choose from fields you've created and reduce work.
- Choose a field type and fill in all required information. I named this example "Home Phone Number."
- If you want this question to be required or accounting related, you can toggle those on at the bottom
- Click "Add Custom Field."
- Now this field is part of your new form. You can add as many additional fields/questions as needed.
- *Note: If you click Existing Fields now, you will see the new "Home Phone Number" field we added
Add Form to Issue Type
- Click into Admin > Issue Types > Choose issue type you want to add your form to
- Scroll down to "Forms"
- You can add a form to appear on Create or on Complete
Choose desired Form and add
Now, when this issue type is selected, the added form will appear on creation or completion, depending on the desired settings.
