Custom questions on COIs will give users insight into contextual information that may have an impact on their tenants’ and vendors’ compliance. Users will be able to standardize sets of questions in a new Forms library, apply those forms to profiles, and document answers on certificates – allowing property management teams to capture details from leases, contracts and policies alongside the proof of coverage included on COIs.
With this feature, you can:
- Configure reusable sets of questions in a new Forms library
- Standardize questions across a portfolio by applying them to Profiles, alongside coverage requirements
- Easily view & answer questions on certificates without impacting COI compliance

Configuring Custom Questions in the Forms Library
- Go to the Insurance module > Forms tab > click “+ Create Form”
- Enter Form Name
- Apply Form Category (Tenant, Vendor, Management)
- Note: Forms can only be applied to Profiles of the same category
- Click “+ Add Question”
- Enter Section Name
- Add Question, select question type, & add answer options
- Note: Users can toggle on “Required Question” to ensure questions must be answered before saving the COI
- Click “Create Form” to save
- Note: Users can delete previously saved forms from the Forms grid
Adding Forms to Profiles
- From the Profiles grid > click “+ Create Profile” or open existing Profile to edit
- Scroll down to “Forms” section > click “+ Add Form”
- Select from Saved Forms in Forms library OR begin New Form creation flow
- Note: Saved Form dropdown will only include forms with the same category as the current profile selected
- Click “Add Form” > click “Save Profile” to save form to profile
- Note: if replacing previous form, new form selection will only apply to new certificates. No changes will be made to forms on existing certificates
Viewing & Answering Forms on Certificates
- From the Certificates grid > click “+ Create Certificate”
- Note: Forms cannot be changed on existing certificates
- Create certificate for Company/Property that has Form on profile
- Note: Default Company/Property profile must be configured before certificate creationGuide
- Scroll down to “Forms” section > Expand All
- Answer all required questions > click “Add Certificate”
Permissions
Users must have the following permissions enabled to use the Custom Questions feature:
- To view Forms tab:
- Enable new ‘Can View Insurance Forms’ permission (never enabled for tenant/vendor users)
- To create new forms & edit existing forms:
- Enable new ‘Can Manage Insurance Forms’ permission (never enabled for tenant/vendor users)
- To view profile with form attached:
- Enable existing ‘Can View Compliance Profiles’
- To add/replace form on profile:
- Enable existing ‘Can Manage Compliance Profiles’
- To view certificates with form attached
- Enable existing ‘Can View Company Certificates’ (allows tenant/vendor users to see form on their company’s certificates)
- Enable existing ‘Can View All Certificates’ (allows PMO users to see form on certificates)
